Service Agreement

This Service Agreement (“Agreement”) sets forth the terms under which Nitty Lice Removal provides head lice screening and removal services to clients.

1. Services
Nitty Lice Removal provides mobile in-home head lice screening and removal using The Shepherd Method.

2. Fees

• A prepaid service fee of $140 for the first hour is required to secure your appointment.

• Travel fees of $1.40 per mile beyond 10 miles from 3130 Balfour Rd., Brentwood, CA will apply.

• Additional time is billed at $140/hour in one-minute increments ($2.333/minute)

• Any refunds, if granted at the provider’s discretion, will be issued minus the entire Stripe payment processing fees. These fees are non-refundable and are not returned to us by Stripe. If a partial refund is granted, the refund will still exclude the full Stripe processing fee.

3. 30-Day Lice-Free Guarantee

We guarantee that treated individuals will remain lice-free for 30 days.

Requirements:

• All household members must be screened

• Anyone found with lice must receive treatment

• Declining screening or treatment voids the guarantee

Coverage:

• Free re-treatment if lice return on anyone who was originally treated

• Free re-screening for household if warranty claim occurs

Exclusions:

• Individuals found negative for lice (new lice = new infestation, standard rates apply)

• Individuals who were not screened during the original visit
  
  

4. Client Responsibilities

• Ensure all household members are present for screening.

• Notify Nitty Lice Removal of allergies or scalp conditions before treatment.

• Provide a safe and appropriate service environment.
  
  

5. Liability Limitations

• Nitty Lice Removal is not liable for:

  • School/work absences or other indirect costs.

  • Emotional distress or stigma related to lice.

  • Re-infestations from external exposure.

• Maximum liability is limited to the service fees paid.

• Services are not a medical diagnosis or treatment.
  
  

6. Consent
By checking the consent box on your onboarding form and submitting prepayment, you acknowledge and agree to this Service Agreement.

7. Text Message Consent

Client authorizes Nitty Lice Removal to send transactional SMS (appointment confirmations, secure Stripe payment link, reminders, and updates) to the mobile number provided. Msg & data rates may apply. Client may opt out by replying STOP; reply HELP for assistance. Consent to receive texts is not a condition of purchase.
